Ankylosing Spondylitis

Overview

Ankylosing spondylitis (AS) is a chronic inflammatory disease that primarily affects the spine and sacroiliac joints, causing progressive pain and stiffness. Over time, chronic inflammation can lead to the formation of new bone in the spine, potentially causing sections of the vertebrae to fuse together, resulting in reduced flexibility and a hunched posture. AS typically begins in early adulthood and is more common in men. Symptoms often include chronic lower back pain and stiffness that improve with exercise but worsen with rest, particularly in the morning. The condition can also affect other joints, the eyes (uveitis), and in rare cases, the heart and lungs. Modern biologic therapies have dramatically improved outcomes for patients with AS.

Common Symptoms

Chronic lower back pain and stiffness
Pain that improves with exercise, worsens with rest
Morning stiffness lasting more than 30 minutes
Reduced spinal flexibility
Fatigue
Eye inflammation (uveitis)
Pain at tendon and ligament attachment points

Treatment Options

NSAIDs for pain and inflammation
Biologic therapies (TNF and IL-17 inhibitors)
Physical therapy and regular exercise
Posture training and stretching programs
Regular monitoring with imaging
Coordinated care for extra-articular manifestations
